- Assists individuals in
gaining access to state funded services, Medicaid
services, as well as medical, social, educational and
other services
- Monitors consumers
treatment plan and services
- Locates and
coordinates sources of help so the individual utilizes
both natural and paid services

Case Management
The Beaufort
County Developmental Center, Inc. provides Targeted Case
Management to children age 3 and older and adults with
developmental disabilities.
This service assists
the consumer, parent/guardian or individual in obtaining services that
are designed to empower consumers and/or parents to achieve or maintain
optimum social, psychological, and physical functioning by arrangement,
linkage or integration of multiple services as they are needed or
desired, or being achieved, by the consumer.
- Case Management services
and supports are available to families, adults and/or children with
developmental disabilities.
- Services may be either
long or short term, comprehensive and determined by the consumer
needs.
Targeted Case
Management is directed toward facilitating increased functional skills
in the areas of self help, language, reasoning, mobility,
self-direction, capacity for independent living or economic
self-sufficiency for individuals with developmental disabilities.
